HSC204 Chronic Conditions Management

This is a specialist elective unit in the Bachelor of Health Science for the Health Promotion, Indigenous Health and Community Health Development streams. It provides students with an overview of preventable chronic diseases, their prevention and treatment, for example Type 2 Diabetes, Renal Disease. Theories of self management are critiqued in relation to efficacy and patients' views. Health behaviour change models such as the Health Belief Model, Transtheoretical (stages of change) Model, and Social Cognitive Theory are introduced and reviewed from an Indigenous and non-Indigenous perspective.
