PHC511 - Chronic Condition Management in the Primary Health Care Context
This unit focuses on the management of chronic conditions in the primary health care setting. Students will examine the management of chronic conditions in primary health care in the context of contemporary cultural, environmental, political and sociological influences. Furthermore students will explore multidisciplinary care and self-management, including self-care that aims to enhance self-efficacy. Different chronic conditions such as cardiovascular and diabetes, as part of the national health priority areas, will be studied along with others. The unit sanctions for student to focus on chronic condition/s that are of special importance or interest for their practice.
